Training Attendance Request Form
This form should be completed and submitted by all individuals seeking to attend an upcoming training class. Completing this form does not guarantee registration, but does constitute the first step in the process.
A purchase order or credit card information must be supplied to Connected prior to receiving a training confirmation. The required information can be faxed to 508-879-5794, attention: Training Department.
Upon confirmation of proper payment method, a Training Confirmation document will be emailed within 3 business days of registering with date confirmation, hotel recommendations, and directions to the training facility.
Training attendance will not be allowed without proper payment arrangements prior to class.
